How Do I.... Register With The Practice?
The Practice Welcomes All New PatientsTo register with the Practice you will need to complete a registration form and provide evidence of identity and UK residency. A driving licence, passport, birth certificate, utility bill, council letter etc are all acceptable forms of evidence. If you have difficulty in providing these documents please advise reception and we will assist you. Further information is available from reception. How Do I.... Change My Details?
We ask that any changes in personal details such as name, address, contact
telephone numbers etc are communicated to the Practice promptly.If you move to an area outside of the practice boundary you will need to register with a practice more local to your new address. Should this be the situation for you the Practice will advise you. |
